From Travel Right:

3. Travel Health Insurance of EUR 30,000 minimum coverage (with 15 days of allowance beyond the travel period) and valid throughout the Schengen area for emergency hospitalization and repatriation expenses. The insurer has to be from the following list of accredited companies

4. Round trip flight booking confirmation

5. Hotel booking confirmation

6. Daily travel itinerary

From Applicant:

1. Long brown envelope

2. Two Photo 35mm x 45mm, passport sized colored pictures with a white background (must be taken within the last 3 months; do not staple the photos on the form)

3. Original passport (must be 6 months valid beyond intended stay and must have at least two blank pages)

4. Original Previous Passport

5. Copy of passport data page

6. Copy of previous visas and arrival stamps

7. Original PSA Birth Certificate

8. Original bank certificate with receipt and addressed to the embassy and bank statement for the last 6 months transactions

9. Copy of asset under the applicant's name

10. Copy SSS contribution

11. Original PSA Birth Certificate of child (if applicable)

12. Original PSA Marriage Certificate (if applicable)

13. Evidence of residence-Copy of property ownership or lease agreement or statement as living with family

Supplementary Documents:

For Employees:​

1. Original employment certificate with leave of absence (must include applicant’s position, date hired, salary, date of return to work)

2. Pay slips

3. Copy of PRC/IBP ID (if applicable)

4. Copy of company ID

5. Copy of recent personal income tax return

For Company That Will Shoulder The Trip:​​

1. Guarantee letter from company

2. Original bank certificate and bank statements of the company (latest 6 months)

3. Copy of business documents and ITR of the company

For Business Owner:​

1. Copy of business registration (DTI or SEC with General Information Sheet)

2. Copy of business permit or mayor’s permit

3. Copy of BIR Certificate of Registration

4. Copy of income tax return

5. Copy of financial statements

For Students and Minors:​

1. Copy of Proof of enrollment

2. Copy of school ID

3. Leave of absence from school (if travelling while classes are ongoing)

4. Consent form (signed by both parents, allowing the child to travel if parents are not travelling with the child)

5. DSWD travel permit (for minor)

For Retiree​

1. Copy of senior citizen card

2. Copy of retirement documents

For Foreign Nationals:​

1. Non-Filipino passport holders must show evidence of long stay permissions for the Philippines, i.e. Visa, & ACR with permission to stay for at least 3 months upon return from Ireland.

2. Copy of international card of residence (ICR)

3. Copy of Re-entry permit

If you have a sponsor:

1. Notarized affidavit of support

2. Bank certificate

3. Bank Statement (at least 6 months)

4. Proof of relationship (e.g., Birth Certificates, Certificate of Employment, Letter of Explanation, Pictures Together)

5. Business / Employment / Retiree documents of sponsor

If you have an inviter

1. Invitation Letter

2. Copy of Government ID of inviter (with signature and address)

3. Copy of Identity page of passport and (if non EU national) valid IRP card

4. Copy of Proof of Citizenship / Residency (if applicable)
